Empower all your patients to understand the importance of prevention and the active role they play
Active prevention requires working together with the patient to identify exactly what you want to achieve. This will help the patient easily understand their active role, allowing you to set achievable goals together. Everyone, including healthy patients can benefit from personalised guidance and taking ownership of their own preventive care.
The following materials have been developed to help your practice implement patient-centric active prevention both at chairside and at-home.
Implementing active prevention in your practice
Give patients an opportunity to ask questions and share any oral health concerns by using the pre-assessment active prevention questionnaire
Remember interpersonal relationships are key, as patients are motivated by the communication they receive from their dental professionals1

Pre-assessment questionnaire
This simple pre-assessment questionnaire provides an opportunity for patients to pre-think any questions or concerns they may have around their oral health. Their answers will also help facilitate a dialogue tailored to their individual needs and concerns, starting their active prevention journey.
This dialogue can help further build the interpersonal relationship between the dental professional and patient, making patients feel more valued and understood. The printed questionnaire can be completed in the waiting room prior to the patient’s appointment.
Patient take away guidance sheet
Implementing active prevention can be a challenge. The active prevention chairside guide aims to help dental professionals engage with patients to quickly deliver tailored advice, motivating patients to improve and maintain good oral health at home.
Use this sheet to clearly mark areas of focus for your patient identified during clinical assessment. This will be a useful prompt for patients to use as a reminder at home.
Patient information leaflet

Follow-up active prevention messaging at-home
Good oral health habits are formed over time, often taking up to 60 days to change a behaviour.2 Following up with a personalised reminder can support patient motivation to encourage them to continue with positive oral health behaviours. You can follow up with patients using a telephone call. A short SMS or WhatsApp message can be used as a personalised follow-up reminder. Emails or communication via a practice newsletter can also be used to share oral health advice along with links to additional materials.
